So, you want to come to camp? Great! We can't wait to see you! Below are four forms that must be completed in order for you to attend camp! Please have an adult complete the forms.

The first form (All About Me) is a fun way for us to get to know a little bit about you! The form (and a picture if you provide us with one) will be given to your companion before camp. The Camper Registration Form provides us with all necessary contact information and permissions. The Camper Medical Form must be completed by your oncologist (or family doctor if you're in remission). The last form, Camper General Release and Waiver, must be signed by your parent or guardian.

These forms must be completed in order for you to attend camp. Download the forms, then print them off and fill them in! Once they are completed, mail them to:
